Appointments
We know you have many choices when choosing a Dentist in Holland so we have made requesting an appointment a simple process via our Website. If, for any reason you cannot keep a scheduled appointment we ask that you contact us within 48 business hours of your appointment and we will be happy to help you find a new appointment time.
Insurance and Billing
We are ONLY providers for Delta Dental Premiere, however, we also accept and participate with most other insurance plans. Please keep in mind that although we submit all claims to your insurance company, we do not have a relationship with them. Our relationship is with you, the patient. If for any reason there is a problem with your insurance, it is your responsibility to follow up with them.
Payment Options
Without Insurance: We offer a 6% discount if you pay the day of service with Cash or Check. (6% discount does not apply to Bleaching supplies, Dental products, and/or Electric Brushes)
With Insurance: If your dental services have been Pre-Authorized and your co-pay amount has been determined then your co-pay is expected on the day of service.
We accept all major credit card payments (MasterCard, Visa, Discover, American Express or CareCredit)
Monthly Payment Plan: if you would like to leave a credit card on file, our office would be happy to make a monthly charge the first Friday of each month.
It is your responsibility to contact our office with any card changes. If your card is declined after 3 separate business day attempts, a $40 charge will be applied.
LATE CANCELLATIONS / FAILED APPOINTMENTS POLICY
Your visit to our office is important. Failure to notify our office of the cancellation of your appointments 48 business hours in advance. If you fail 2 appointments in a row, you may also be charged a failed appointment fee of $50 for hygiene appointments and $75 for doctor appointments. If either of these occurs, we will reshedule an appointment for you until balance is paid in full.
